F L U I D I N , F L U I D O U T W e constantly struggled with getting physi- cians to update H&Ps in pre-op and to complete their immediate post-op notes. Adding attention-grabbing tabs to patients' charts helped a great deal. We use bright colors to draw attention to the 2 forms that surgeons need to fill out: the H&P and the procedure notes. It's a pretty simple concept, but we've achieved great results.
