Tubular retraction systems emulate the "keyhole surgery" access of abdominal single port laparoscopy in order to sidestep muscle incision and give healing a head start. This retractor, inserted through a small inci- sion, pushes the muscle fibers aside as it enters instead of tearing through, then dilates to expand the hole it's made. The 18-mm tube allows the entry of fusion or discectomy instruments or implants, the removal of bone or disc tissue, and magnified views from the operating microscope. Then, once the tube is removed, the undam- aged muscle fibers just slide back into place. Extreme lateral interbody fusion, also known as XLIF, accesses the anterior lumbar spine from the side.
