plete their online medical histories and fill out other forms — at their
convenience — before arriving in our center on the day of surgery.

• Automated calls. As soon as patients get on the surgery schedule,

go to our website and fill out the forms online. If they haven't done so
as we get closer to the surgery date, they'll get another reminder call
(at this point, a pre-op nurse also calls patients to see if they need
assistance).
