Our healthcare system is also incentivizing value-based care and penalizing providers for high rates of medical errors. The employees who thrive in a patient-safety focused work environment may already be in your organization. Are they recognized and supported? Do they feel comfortable pointing out unsafe practices? Does everyone who sets foot in the building share their no-excuses mentality? Until the answer to all of these ques- tions is yes, health care's silent killer will continue to prowl with impunity.
