Establish a culture of safety
Base medical interventions on a process, not human heroics. Give your
nurses time for truly effective patient assessments. If they feel uncom-
fortable with how patients are handled, be supportive and brave enough to facil-
itate the necessary changes in your facility's policies and procedures. You have
an obligation to your patients and staff to facilitate an environment of safety.
The accreditation bodies may not require root cause analyses for negative out-
