bag can be a significant and serious complication
that can jeopardize the patient's vision. We have
eliminated phaco and its associated dangers from
99% of our cataract cases with a little help from the
laser (see "2 Steps to No More Phaco").
My phaco-free technique can only be accom-
plished safely when the lens is flowed out of the
capsular bag and into the anterior chamber, where
minimal or no phaco energy is used. The technique
isn't innovative, but performing it in the anterior
chamber demands great skill and careful movements
to avoid causing cornea injury. I perform the 180-
degree opposing-action lens fragmentation under a
thick layer of viscoelastic placed underneath the
cornea's dome to limit that risk.
Positioned for the future
The cost of laser platforms will eventually decrease as
new generations are developed, but there's no arguing
that adding the technology to your facility in the cur-
rent marketplace requires a significant capital pur-
chase. Deciding to invest in the future of cataract sur-
gery depends on your case volume, but also on the
goals of your surgeons. Are they content in removing
clouded lenses with manual surgery that can't get
