1-Page Form Accompanies Patients to Hospital
During our combined state
and Medicare/Medicaid
inspection, our surveyor
dinged us for transferring patients to the hospital without a note from either the surgeon or anesthesiologist in the medical record
addressing the decline in condition that warranted the transfer.
We've since created a 1-page patient transfer form
(www.outpatientsurgery.net/forms) that summarizes all the pertinent information. The top half of the form is the nurse's summary. It gives a snapshot of the patient at
the time of transfer and pertinent transfer info, as well as the drugs received in the OR and PACU.
The bottom half has space for the surgeon's or anesthesiologist's
transfer note.
