Page 20 IDEAS THAT WORK TRANSFER NOTE Be sure a physician addresses the reason for a hospital transfer on your patient transfer form. TRANSFER NOTE 1-Page Form Accompanies Patients to Hospital During our combined state and Medicare/Medicaid inspection, our surveyor dinged us for transferring patients to the hospital without a note from either the surgeon or anesthesiologist in the medical record addressing the decline in condition that warranted the transfer. We've since created a 1-page patient transfer form (www.outpatientsurgery.net/forms) that summarizes all the pertinent information. The top half of the form is the nurse's summary. It gives a snapshot of the patient at the time of transfer and pertinent transfer info, as well as the drugs received in the OR and PACU.
