you identify
patients who are
at increased risk
of suffering pres-
sure injuries.
Here's how to use
it:
1. Review
patients' records to complete the information in the left column.
2. Place a check in the right column if the answer to the ques-
tion in the middle column is yES.
3. Two or more yES answers indicate an increased risk of pres-
sure injury.
This form provides real-time identification of a patient's risk for
pressure injury and lets your staff know when to implement addi-
tional preventative measures.
