loading devices elevate the heel off the OR table, distributing the
weight of the leg along the calf without putting all the pressure on
the Achilles tendon. Be sure to position the knee in slight flexion
when offloading the heels. Apply a layered soft silicone foam dress-
ing on the sacrum and heels to decrease friction and shear injury
risks. (Protective dressings do not preclude the use of positioning
devices.)
• Lateral. The dependent ankle, leg, knee, hip, humerus and side of
the face and ear are at risk. Pad under the down leg and support the
back from just under the arm to the top of the thigh. Place a pad
under the arm that's extended on the lateral armboard and make sure
padding sits between the legs.
• Prone. Protect patients' toes, patella, thighs, rib cage and ears.
